I don't recall ever hearing details of the arrangement, either: only that there are 4. My guess is that artists who like Krogans but don't like the 4-testicle anatomy (or need an excuse for why they forgot it) started the headcannon that two are internal in order to excuse their art not matching the in-game description.

If I remember right, it only states it after you've beaten the right of passage mission where you kill the Threashermaw in the arena. When you walk around after you'll hear some npc's say something like "He/She has a set of quads. Only one to beat a Threashermaw alone was Urdnot Wrex." Or how ever it's spelled. It's been a while since I've played. :)
