So, those red dots are the eyes, as you might know if you've followed me along drawing these for a while, like https://www.furaffinity.net/view/19468438/, https://www.furaffinity.net/view/20740543/ and https://www.furaffinity.net/view/21128069/ . They can extend them like snails to see better. The skulls, furs, etc aren't actually part of the creature itself: the creature is just a semi-liquid, boneless darkness with glowing eyes which they can carry inside their bodies when not in use.
They find/steal parts from dead creatures and wear them, holding all of it in place manually and thusly they might drop parts when startled or upset. They slowly grow more eyes with age, and get more proficient at holding parts in place, and so they can hold/wear more antlers/claws/furs. So older ones might wear several 'heads', and have twenty claws on every 'paw', whereas younger ones might only have a garden snail shell and a single eye. They will sometimes fight one another over parts they find particularly attractive, hoping to tear them away from the other or just scare them enough to let go.
That tiny garden snail youngshade is desperately holding on to its shell in order not to accidentally drop it or crawl out of it. <3
