If I remember correctly, the most popular place for the off-arm to go is behind the back, or on the hip.

Sometimes it is also placed flat against the chest to allow faster grappling.

According to actual manuals, that is correct. The empty off-hand is better to be placed behind the body or rested on the hips in order to prevent it from being targeted. However in most depiction, a rapier strike is usually indicated by their hands hovering up and then stretched out during the thrust from the other hand. It adds the dramatic effect~ and I wouldn't say it's a good pose in a real combat :P
