Belgium always have been the most logical alternative route for then European superpowers (German and France) to invade each other, instead of more direct route through Alsace-Lorraine, which had exchanged ownership since forever. So it is quite logical too for them to build some of the stronger forts in Europe. I think the one you mentioned is the fortified positions of Liege, on which Germans fielded 38 cm coastal mortars, 42 cm Big Bertha and a zeppelin to bring them to surrender. During WW2 these forts (including the newer outer ring forts - one of which was the famous Eben-Emael) also became major speedbumps to troops crossing Belgium, Axis or Allied alike

The gun has ammunition trolley to haul shells and charges around, similar to this one:

The shell is raised to breechblock with lever powered by hand cranked hydraulic jack, which also powered the rammer
As for accuracy, it is usually employed as area effect weapon equipped with HE-FRAG shells, but with APCBC shells and good maintenance it could smash down medium sized fort with good repeatability

Which means that with this kind of reloading procedure, the fire rate must be pretty slow.
Regarding the HE-FRAG shell, tried checking the Wiki, it just redirects to normal HE. I'd imagine that HE-FRAG is just like HE, but with shrapnel (as the name suggests) splintering in all directions? An anti-infantry shell basically?
And APCBC are basically bunker-busting shells, right?

True, this gun has average rate of fire of about 6-8 round per hour
HE FRAG shell here is basically a HE shell that have indentations on the inside to promote fragmentation to forward direction. it's not specifically anti infantry though, as the larger fragments can reach tens to even a hundred kilograms in weight and quite capable of disabling medium tank from top.
