It depends on the dragons, really. Smaller, egg-laying dragons usually gestate relatively smoothly, the 'worst' part being the eggs feeling odd as they shift about in the womb. Live-bearing Asian dragons take ages and ages and AGES to gestate, but they're so wriggly and noodly as they grow, and usually don't get tooooo big. Larger egg-bearers pose a problem, because large eggs, while fun to carry, are hard to lay. The large, live-bearing western dragons though. Those are fun, but difficult. They get so big, and so active, trying to spread their wings and such while in the womb. They also take a long time to gestate, but that's in part because they seem to actively avoid birth. They oftentimes become overdue, and since many dragons love heat and darkness, and tend to resist when contractions start up.
