It's a fetish; the only way to understand it is whether you have it or you don't, and there's no exception to that rule.
I can tell you this though; macro is a multifaceted fetish. This means that it's appealing for a variety of reasons and the fetish in of itself covers another range of interests. Though it's not required, macro often carries themes of vore, feet/paw, guro, domination, etc, which aren't mutually exclusive.
The prevailing theme is the sheer size of the subject, which brings in aspects of hopelessness, awe, voyeurism, powerlessness, etc. "Death of tiny ants" does not reach the mind of a 'god,' so to speak, so yes, it's pretty mean, but that's partially the point.
