Highlights: Large airbrush, stupidly low opacity (like 12% or less), full white and many many strokes. Full black for shadows.
Apply highlights furst, then shadow on the opposite 1/3rd.
And personally I have shakey hands so my stabilizer is always set to full 19, halps make the curves better too.
