His mom used to beat him up when he was younger. Whenever she would have a bad day, she would tie him down on the bed and smoke a cigarette ..When she was close to finishing her smoke, she would turn his feet toward her and put her cigarette out on the sole of his foot. . He has polka dotted feet now. ..He is very sensitive about that.

This looks like he is wearing a medical gown. This is probably after he has been hospitalized for his mental break. It starts with his bipolar and then it breaks or he has small breaks in his mental state.
