I make them fully in Zbrush and then import them into blender for rendering. I do make some parts in Blender if it's easier than doing them in Zbrush, but I'm getting better at keeping it mostly in Zbrush.
For this model I made it entirely in Zbrush and then took it into blender and retopologized it and rigged it up for rendering.
