Requires a successful grapple check!!! Then again, sometimes you go to grapple a prone/unconscious bandit, and, wouldn't you know it, they wake right up and throw a fit. The nerve of some people, right?

Actually, the problem comes from someone swallowed possessing a light piercing/slashing weapon. You're AC is negligible from inside and they don't need to do that much damage to cut their way out. If they lack said type of weapon, then they're possibly hosed, but otherwise it's not hard to get out, just a turn or two of stabbing the stomach.
