While very good questions, I admit I haven't really thought this through - it was done in one day after all. XD
I just assumed that the future would be very strict in training and putting people in stressful situations gets them used to adrenaline frequently and make them adapt better as fighters. Also, futuristic boxing might be even more lucrative than present boxing, so I guess anyone crazy or greedy enough would give this a go. XD
And anyway, this is sci-fi, all speculation-based. I'm just guessing how boxing could get more exciting in the future, so I extrapolated a few things my way. :P

The term "Underdog" comes from an old "sport" called Bear Baiting.
Basically an acctual Bear and 2 dogs are put into a pit.
The dogs are training to attack specific parts of the bear. The "Top dog" was trained to attack the bear's upper body and head. The "Underdog" was trained to attack the bears lower body and legs.
Anyone will tell you that if you try to go after a bears legs in a fight is near suicide because the bear still has it's front paws not to mention jaws that could rip you to shreds.
People would bet on the dogs to see which would kill the bear first. The underdog, being at a severe disadvantage based on how he was trained, was given worse odds for betting on the fights (the top dog woudl be given something like 2:1 or 3:1 odds, while the underdog woudl be given somethign like 10:1 or 15:1). If the underdog won, anyone betting on him would make quite a good bit of money, because the dog had to work so much harder to get the kill.

It's remarkable that someone could be as foolish as to get in the ring with that monster of a machine. Sure, if you were a fairly strong fighter it might be a good idea, but otherwise you'd either have a death with, mentally damaged mind, or just really need money. No matter what the case, it's going to hurt like hell for a while.
