1901 ALCo 4-4-2 "Atlantic Camelback" Central Railroad of New Jersey
Camelbacks, Mother Hubbards etc had their cabs right above the wheels so that the width of the firebox wasn't constrained by the width of the cab and crew. This gave them a huge increase in how much steam they could create. For a time these style engines were some of the fastest around.
For the crews, however, they were miserable machines.
The fireman really didn't have any cab to protect them as the firebox went to the very back. The engineer had the reciprocating assembles right below them which in a rod failure meant pretty much death from the flying members as they flung around.

This type of locomotive was eventually banned.
