Manually, in photoshop/photopaint.
When you make a b/w anaglyph, you convert photos to monochrome, then combine channels -- image for left eye into red channel, image(s) for right into green (and blue, if you're going for cyan filter). Adjust position/brightness/contrast to taste.
Making color one is a matter of replacing the red channel in the *color* image for right eye with only the red channel from the image for the left eye. Again, adjust as needed.

I don't think there's such thing as glasses being off -- they are just filters.
And the red/cyan are that far apart sepecifically for the whole scene in front of the mesh to "come forward" from the screen, not vice versa. It's just the matter of focusing on the right spot. Try looking at the mesh, then the flowers. I'll make another one "fading back" to show the difference.
By the way, did you really like this one best? I personally like the first one MUCH more than these two.
