I found this evil outsider in one of the Pathfinder bestiaries called an Utukku. It's an elder demon type of creature that steals your humanity so it can impersonate you and infiltrate society. I loved the idea, but the artwork the book had for it looked too humanoid for my tastes, so Schnops and I redesigned it.
And here, Kestra has summoned the creature because she's fixated on one-eyed beasts and is always trying to find the secret of eternal life and beauty from outsiders. Well, she screwed up the summoning process and the monster escaped. Now it's latched onto her with one of its sucking mouths and is beginning the process of draining her humanity. And what will happen when she's got no humanity left to drain? Well, you'll find out in the next pic.
