About a month back or so, I got back into playing Cry of Fear again, having not played it for like 4-ish years since, and learned very quickly that I had lost all my saves and unlocks during that time not having it installed. So I did a regular casual run, and after that I decided to try and go for the unlockables again. The ones I aimed for in perticular were the weapons. Namely... the nightvision gasmask, the axe (most damaging melee weapon), the camera (permanently freeze non-boss enemies), the famas (30-round fully automated gun with infinite ammo clips), and finally the book (extremely high damage to all enemies in sight). The gasmask require you to beat the doctor mode that you unlock by beating the game, the axe require you to deliver a package you get by getting ending 4, the axe require you to input a code of 10 numbers (that are all between 1-50) and then kill an enemy with the axe to permanently have it. The camera require you to beat the game on any ending in under 2½ hours, the famas require you to beat nightmare mode which is harder than hard (and unlocked after beating the game first time)... and finally, the book require you to beat the game with an S-rank. It didn't take me long to get all up to the famas, because 1 I had already gotten them before so I knew where to go and what to do, and 2 because once you get the mask you will have a WAY easier time beating the game in under 2½ hours.
Moved on to nightmare mode, and it's no joke. If you want to beat this difficulty you're almost required to have all non-nightmare-unlocked weapons! Plus all enemies has far more health, do TONS more damage, AND you can only save a max of 5 times! Those save slots are very critical you use sparingly, because it's not a short game to say the least, and due to it being on the half-life 1 engine it's prone to bugs and random crashes. Plus since you have to no-damage the whole run essentially it's easy to fuck up at critical points, so you need those saves bad. (ESPECIALLY with the enemies with the guns, the suiciders. They hit 40% of max health damage per shot on nightmare and on that mode they gain the ability to headshot you, instant death!) Despite this, I decided I wanted to try and get my first run as an s-rank. To get an s-rank, you have to meet these criterias:
90+% gun accuracy, max 4 times saved, max 2 syringes used, beat game in under 2½ hours, play on nightmare mode.
First playthrough I did get close, but I fell short by somehow missing 55 shots, ending up at 82% accuracy. It was literally the only thing that I fucked up on! Was bummed by this but went for another go. This time with the famas so I wouldn't have to scavenge glock ammo. After a long run and a ragequit after having died after on the very last pack of enemies in the whole game before the final boss, I beat it and saw the results screen... I was utterly baffled! Somehow despite me trying my absolute hardest to do one-bullet shots with the famas and even killing FIVE sawrunners (highest health in the game, and you need to unload about 4½ clips of famas bullets into him to kill him... or 45 axe swings) I had somehow gotten 43% accuracy! I just couldn't believe it! I was baffled and pissed at the same time! How the hell did I miss THAT many times!? 5 fucking sawrunners! I saw the bullets hit them! Ruby here represent how I felt at that time
I still haven't gotten s-rank, but I'm doing one more run with the famas to determine if I need to switch back to the glock or not...
EDIT: In hindsight half a year later, I think it was because bullets hitting sawrunner probably doesn't count...

That's the thing. In regular playthrough you can never kill one unless you plan very very carefully and move with flawless execution. Because each one has 2000 health, and a single bullet from a glock does 13 damage, while a knife does 6-9 damage. So they're literally meant to be avoided and serve as chase sequences. Hell, I personally can only kill them if I freeze them with the camera! Which is how I've killed them at all. I then crouch, aim at the torso and fire away! I crouched because I figured the chainsaw would block the bullets like the Faster can block bullets with their spike arms.
I don't get it either... I can see the blood spurt from their bodies and they make a sound which signal them getting hurt
Oh and the sawrunner is the only boss that you can freeze with the camera, all others are immune... and 3 normal enemies that have consistently bugged out on me
