I bet that old Asami did something to the mask to ensure that the evening was ruined. Timed firebomb. bit of tinder, soaked in wax or fat to burn slow and hot. And a smear of something like lacquer, covered with hard wax, or very thin porcelain to stop it catching until it burned down enough or got hot enough to burn through.
http://www.furaffinity.net/user/phantom.berserkr is who recently broke their wrist in a car accident, but she has nothing to do with the production of Blade Under Mask, she just happens to be someone WhiteMantis knows, and Mantis might have let people know about the accident in order to help Phantom out. I have no idea how FurBrowser came to their conclusion.
