I don't know what looks more painful, thedesk or the way you hold the stylus I sit with my legs crossed so my arm always rests on my leg, but I tried to hold my pen like that and I got sore after a couple of seconds, though I could see where that could maybe give you better control for brushstrokes!
You might could use some sort of tilt-table, like small artist's drawing table?
Or there's an old calligrapher's fix: You get two small drawing boards (or panels) and attach them together on one edge with a length of piano-hinge (which you should be able to get at a hardware store). You can then open and adjust the boards to an angle that is comfortable, and prop them up with a block of wood between. That can be sitting on the edge of your desk, so your forearm never touches the edge. Maybe you might have to attach a strip of wood to keep the tablet from sliding down.
