Yup, he´s like that little rabbit in Alice in wonderland, but with added damage to his lung and spine.

He might be unjured, but hes a serviver, and learning to live with a serious back injury can be very hard, soo soo easy to move the wrong way and screaming in pain really does not help with the pain at all.

oh I missed the part about the lung that's even worse

Yup,
When the tidekiller chomped and crunched him, he got puncture damage to both his spine and lung, which never really healed 100% correctly.
So it hurts really bad whenever he exhaust himself too much or bend a way his spine doesn't like.
