Image tag time!

I didn't like the original design one single bit (done by JollyJack of deviantArt.com) so I redid it as what I thought it should be done.

I tag: YOU! After completing this, tag 3 other people (the good 'ole Left4Dead 4-player way ;P).

Simply fill in the boxes with content related to the heading of each box, then place a blacked-out cutout of your character in the space below 'ALIAS'.
