I just had an interesting, if grim thought.
What if this dark horse is not so much a sibling or a cast-off piece of self but is Ciel's old boyfriend? If so maybe the reason Ciel is 'invisible' to others is because this dark horse made him feel that way; like he didn't event exist for anyone else. Not in a good way either, perhaps, but in a controlling and abusive way? Ciel's expression in the bottom left panel is kinda what leads me to this possibility.
