Pearl's Dogmeat is a bit different than your typical german shepard. The big Hellhound took what started as a pup into her care after having to slaughter the pup's pack because it was being rude to her. Pearl's not exactly the 'cleanest' doggo around. She's immune to radiation as all hellhounds are so that forced the pup to live with a constant feast of radiation seeping into her.

Lucky for the pup, the years of rad exposure didn't kill her but forced her to grow, and grow. At first bigger than any of the pack she had come from and then big enough to be nearly shoulder high to her owner. Let's not forget the extra heads. That just happened and Pearl isn't gonna complain having 3 cute doggo friends instead of just one. That isn't to say of the other obvious benefits the mutations caused.
