(sighs) I knew this one was going to cause confusion, now this is involved, so bear with me...
The "scarred side" of the coin references Two-Face, a Batman villain known to wear suits (often seriously mismatched on purpose to go along with the dichotemy theme) who flips a two-headed silver dollar with one side scarred top make his decisions for him due to an underlying belief that random chance is the only justice, he has also been known to weild a tommy gun an numerous other kinds of firearms.
In the movie "Moonwalker" which Micheal Jackson starred in as himself, a music video portion of the film (and only decent bit) involved him in this suit and performing the song "smooth criminal" after which he immediately pulled a tommy gun on the approaching bad-guys of the movie, sending mixed messages about his personality and mental state.
Hence the combination of these two themes due to the presence of a flipped silver dollar in the picture.
