Pneumonic plague was (and is) airborne, but it was believed by these "doctors" that the putrid stench was the way of transmission. They masked the odor using aromatics which filled their beak-like masks.
They were brave people that often died, but they practiced pseudo medicine. Not all had great intentions; many saw it as an opportunity for massive gains/profit, as they were in high demand by plagued towns at the time. A great prospect for underperforming medical students and practitioners (they also served as great death counters for statistical purposes).
They sure looked neat, though. :P

My first interaction with plague doctors was in Assassin's Creed 2. Thankfully, they were all kind and didn't try to cut off my arm because it was "infected".
Really great work here, especially with the background and lighting to give off that mysterious and slightly sinister feeling.
