Why do I get the impression she is "making the hippocras last longer" by peeing into the bottle?

Yeah, I had to reread it a couple of times going why is she peeing in a bottle? Just to pick a nit, females did not travel alone.. ever. Females were considered property for the most part and if she traveled alone she was considered fair game and she would be low hanging fruit for any highway men and bandits.
Yeah, I know it's fantasy but women today are rather reluctant to travel alone (for good reason) imagine back in the middle ages when women disappeared without a trace every day. Joan d' Ark had to dress as a man just to be able to move around Europe and had an army to back her up. Few people died of syphilis in their twenties because it has a latent period of 10-30 years in most cases and syphilis came from the Americas so it didn't show up in Europe until 1494 or so.
Women traveling alone still disappear today, even in places you wouldn't consider particularly hazardous. Both of my sisters, who travel on business a good bit, have had dodgy experiences with airport "security."

Skeletons and a few mummified bodies bear evidence that syphilis was around BEFORE Columbus supposedly brought it from America. Outbreaks of the disease occured in armies (such as the French Plague). The body can't fight it. The disease may have evolved through 4 stages and the final evolution has the latent period. The 1st form was a skin disease or rash, but it evolved by the 3rd evolution into a virulent systematic disease. Mayhaps it wiped out genetic lines that could not accomodate it, OR, as you said, the malignant strains could not manage to spread before killing their hosts. Just like Ebola outbreaks ,run out of victims and Polio became a rare disease because so many potential victims were immunized.
