This particular shirt meme is mostly to shine a light on the move from Trump's administration to erase trans people from law (there's a push for a law to redefine "sex", and therefone gender in regarfs to legal matters, so it only takes into account the gender you were assigned at birth based on genitals; they're trying to erase all notions of gender-related issues from the UN; recently all notions of gender and trans identities were removed from official US government websites). Basically, with the moved they're trying to pull off it'd erase trans identities legally, and so trans people would no longer be protected by law from gender discrimination which would endanger trans american's livelyhoods.

I guess I can kind of see why they would want to remove these identites for legal and medical reasons. Like if you get in a car accident and arrive at the hospital unconscious, do you want your records to say that you're the biological gender you were born with OR the one you selected for yourself? People have similar biology, but there are physical differences between men and women (this statement of course doesn't take into account the dozen's of twitter genders that exist outside of biology). If we're talking just a trans person hanging out with their family and friends then by all means said person could have themselves called whatever they want, but in case of an emergency situation things need to be more specific for safety.
Secondly, what you stated is that trans identities won't be defended under law, but if these trans people are simply assigned their birth sex as their gender then they're still covered from gender discrimination. They just aren't identified the way that they want to be, but the law still protects them as a male or female. So I don't think anyone is going to get discriminated against.
Again, in a social or workplace setting (where these genders might be enforceable) if these people have good friends and family then they'll still refer to them as they want to be referred to. It seems to me that this change really just comes down to helping these people in medical and legal situations. Aside from that, there's nothing that a president can do that the next one can't undo in 5 minutes in office. In the long run it's kind of a non-issue. Just wait two-six years and this will be corrected.

1) Your example doesn't really work on this situation; there's nothing that could happen on a car accident that would absolutely need for doctors to know what gender you were assigned at birth. Most medical situations and emergencies don't really call for knowing what your genitals are/used to be.
Since this concerns the USA, let's talk US law. Under US law, you need to be either medically transitioning or have "completed" your transition to change your gender marker on legal papers; this would indicate at least being under some form of HRT (hormone replacement therapy). HRT changes the way your body functions, and can lead to changes in things such as bone density and blood pressure. Trans people need medical professionals who are knowledgeable on how HRT affects them so they can get proper treatment and medical checks of their levels, people who don't just treat them as cis people, since trans people don't have the same needs as cis people. And that's only HRT, not to talk about GCS (gender confirmation surgeries, such as vaginoplasty, metodioplasty, phalloplasty, mastectomy, etc) which require special attention all on their own.
A bit off-topic, but in biology, currently scientists are working on a system that considers sex as a spectrum with more than two categories. The biggest sign of this is, well, intersex people. I'll be dropping a link on this below, since I'm not a biologist myself so I can't go on too much about it
2) It wouldn't work that way. Since trans people suffer from distrimination due to their status as trans, the removal of the trans identity in the eyes of the law would prevent them from getting help when harrassed or affected negatively, under the logic that it would be impossible to suffer negative repercussions for being trans if being trans isn't a real thing in the first place.
3) Not everyone has a reliable support network, and not everyone can be safely out around their friends, family, and/or co-workers, especially after the law goes through (if it does, let's hope it doesn't).
Erasing a person's identity as trans doesn't help at all from a medical and legal standpoint. It just removes their ability to be protected by the law when they suffer harrassment or attacks due to their gender, and the ability to get proper healthcare (because trans people don't have the same health needs as cis people do, whether they be medical or psychological; trying to make them fit in a system oriented only to cis people is very unfair).
Two to six years is a long time. Even if it can be undone, the weight of it will remain. We shouldn't be tolerant of our rights being taken away just under the premise that someone someday might give them back to us.
