This is one of the only stones facing north in the Bell Rock Cemetery in Malden, MA. Most face south.
There's no identification of the deceased at all. Only the following:

"she yts here interd needs no Vercifying
a Vertious life will keep ye name from dying
shell live thouh Poets cease their Scribling rime
when yt this stone shall mouldrd be by Time"

(The y in the first "yts" seems to function as a "th".)
