Actually, in this case, it's a real head, not a pumpkin. The dulahan carry their own heads, which are often contorted into grotesque expressions to enhance fear in those who they've come to harvest. They can be forerunners or death or the direct harvesters there to claim a person immediately. It varies depending on the circumstances and the legend, though usually leans toward being among the ones who actually do the harvesting/bringing of death.
