Almost since Ruffy has existed as a character he was also intended to be a monster. A formless shapeshifter that enjoyed the soft brightly colored life that causes him to adopt his normal look most often. But when needs/wants must he will dip into the Eldritch to scare, feast and/or fight.

He becomes covered in eyes and his mouth zippers open around his body. His wings are actually the tips of tentacles poking out of portals to his abyss on his back. When monsterfied his "wings" extend outward into long pale and translucent white tentacles.
