The thing about Sam is he loooooves overeating. That sensation of fullness that goes way beyond the rational, when his stomach feels like a rock and is sharply painful, and even moving is beyond his ability: that's his perfect state. Though eating this much makes him miserable, he also can't deny that this sort of stomachache has its own sense of pleasure attached to it.
It's for this reason that Thanksgiving is his favorite holiday, and this is the consequence of a particularly gluttonous Thanksgiving feast, wherein Sam tried to outdo his last year's performance.

I think he succeeded. He might be regretting it. He's definitely enjoying it. He probably needs someone to rub that belly. Just...very carefully.
