As a teacher in the Ponyville Elementary School, part of Miss Cheerilee's job was ensuring that all the little ponies under her tutelage were kept in clean, dry diapers, and so she had a large changing table in the back where she could take care of two students at a time. Of course, being an Earth Pony meant that she had to take care of the problem with her mouth. And changing diapers was not the most pleasant thing one could do with one's mouth. Usually the fillies and colts that she changed were able to stay clean and dry for a bit before they needed to be changed again. However, sometimes they ended up like Sweetie Belle: messy as soon as a fresh diaper was applied. It's not their fault, even if the smell can be so bad it makes the other students like Diamond Tiara feel sick. And so Cheerilee just accepts the duty and gets it over with.

Actually earth pony and pegasus (and possibly some unicorns too) use both (mouths and hooves) to change diapers.
They use their mouths to open/close and transport the diaper but they use their hooves to clean. Because ponies pull, tear and carry things with their mouths.
So yes, ponies open the full diaper with their mouth. Really close to the nostrils.
