I think it's due to the fact that they're padded, themselves. This gives a padded caregiver more of an empathetic appearance, than just a normal caregiver. Since they're wearing diapers, they know what it's like. And possibly have some firsthand experience with the various situations that can arise from it (good, bad, and neutral). The logic of that is likely why we like them so much. Plus, if the caregiver is padded, there exists (at least theoretically) the option of roll reversal or of them joining in on some "little time". To twist an old quote to be more ABDL friendly "We have met the caregivers, and they are us."
