Bonus fact - the latter half was based on an actual exchange held in WoW's "Halls of Origination" around 2010.
After several wipes the Priest healer PM'd me (Warlock DPS) saying he wasn't going to heal (Freakin' PUGs...), so we watched them all get imploded. It was fun. Stuff like that happened occasionally but what made it memorable was the tank (Warrior) made the worst pull I've ever seen - even 10 years hence. I didn't know it was POSSIBLE to pull 3 of the 4 bosses; if you look at a map of the instance, the four seats are all in different rooms, separated by a large hallway. We concluded he was such a bad tank that he somehow triggered a bug.
After the tank and Druid DPS died, a Mage DPS congratulated himself for being first on damage. Cue witty reply from the Priest and half-written reply from the Mage. He'd forgotten we couldn't move with wasd when typing. After they left, the priest thought I'd leave too, but we ended up finishing the instance with a new tank. xD

That's certainly a memorable one.
I had enough of those moments particularly in WLK when I played with my brothers, especially with the one who was a Paladin healer. But nothing quite so dramatic that I can remember anymore.
Only, I remember one time doing Pit of Saron with my Paladin brother and he got so annoyed at one of the damage dealers consistently pulling aggro for no reason he simply whispered to me "I'm not going to heal him" and just didn't for the entire run. I'm pretty sure the DPS left and we just got another one.
I guess being a healer is pretty thankless at times. Though to be honest, I played all roles as different characters and never felt the need to be an ass about people performing badly; as long as a run was completed and didn't take overly long, I didn't really care if people were being dumb. :P

One of the things I liked doing was soloing everything possible. Near the end of wrath until the middle of cata I even tanked a few bosses in heroics. HoO wasn't one of them but I'd taken over for the tank a few times in other cases - particularly when they died. In the Halloween special that year I tanked the entire instance because the tank was a haughty jerk out the gate. Instantly after we entered he said something to the effect of "I'd like to see you all do this without me." So we did. He actually stayed 'til the boss was down and played his DK as a DPS while I tanked using Soul Link. After we downed the boss he apologized and said something about his sister, I'm kinda fuzzy on the details by now. I remember not really caring because we were already done.
As for the HoO run, that tank was ludicrously bad, I was surprised the healer put up with him until the last four bosses. He was wearing mismatched blues and PvP gear along with some shaman items. He had a knack for pulling everything onto himself, running directly away from the healer's range, dying, and then ranting at the healer the entire time he was dead. Like the entire time. He'd even keep ranting after being rezzed while the bosses were still hitting him. This dude was an enigma.
You know, people not dropping threat surprised me because, eh, for me at least it seemed aggro management was basically a non-issue. I've heard some variation of your brother's story probably dozens of times. I'd think, just watch recount and drop aggro when needed. It's not like I had bad gear; I was usually top DPS. *shrug* Maybe it was because I played Demo which split damage between player and pet. Or it could be because I had a UI full of warnings and timers, threat being one of those.
I too played as a tank and a healer. I had a DK tank alt and never had any issue holding aggro. Only when I was on my priest alt did people seem to pull aggro, but even that wasn't too common. I remember pulling threat once while I was leveling my priest, I dropped threat, the boss ran back to the tank while he was typing to yell at me for pulling threat. Then he lost threat to a DPS because he was busy typing. I healed them both and he was pretty nice after that. I got thanked a lot on both my DK and priest so I rarely felt people were overall mean or rude. I mainly heard nasty remarks on my DPS, and then usually directed toward someone other than myself. Only once did someone really go after my DK; he started ranting about how all DKs are awful tanks, etc, after he died standing in front of the first boss's charge (which, if I recall, was literally called Unstoppable Charge, lmao). He did that for two bosses. Until I duoed the third boss with the healer as the DPS all died standing in fires (not enough DPS for the enrage, though) He apologized (while still dead) and when they ran back we completed that boss. Sadly that run didn't finish because two of the DPS ragequit after the healer refused to heal them. We replaced the DPS but then the healer had to go and there were no replacements 'til I had to go to a doctor or something.
