It's mainly in the details and what you choose to represent on the map. Effective maps use different (but carefully chosen) typefaces to denote different types of information (bodies of water, territories, cities, and so on). It's the same in a fantasy setting or in science fiction- too much detail and you lose coherence. If you're creating a map for a story, I suppose it goes without saying that you should consider the path/s of your character/s and logically lay out the towns and landmarks accordingly.
