There is a nanobot-based virus in the blood and body of all living Martians. Repeated exposure to martian fluids, as in, repeated sexual activity, can cause a mutation.
There are other methods too. They can inject you with the nanobots directly, or they have transformation machines. That's probably what happened in this picture, seeing his surprise. The transformation must have been sudden, and over night.
