Uh you clearly do not understand what I said. That was the point, to make it more obvious he's NOT doing his job. Since he's saying "Healing? Was ist das?" But technically...he's healing! I was saying if he healed an equalizer holding Soldier, it would be way more clear what the joke is and how it relates. :)
Well, the image is being used as a spray, for one person.
It would be great to do something like that if it were going to be a single, normal picture, but doing multiple characters in an image that ends up approximately 256 x 256 just makes for a crowded mess as far as making the point of the image clear.

The point of the spray was a medic who was completely ignorant of what their job was, and had just found out the glowy end of that weird leafblower smells funny. The similarity to the taunt wasn't entirely intentional. I was given the quote by the person this was drawn for, and worked with it.
Bear in mind also that the sprays I make are transparent. With sprays being untestable in-game at the sketch stage of production, it's easier to get one character right the first time, than over complicate something that doesn't need to be over complicated with redoing multiple characters and filling up the person's (or my own) spray menu with failed sprays. Why would I complete a spray with multiple characters, only to find out I need to redo it from scratch because the text doesn't fix properly between characters, or important details can't be made out because the characters were made too small?
Not to mention I do these for fun, and they're free, and if people like them why complain?
