The reason distractions work is NOT because "it hurts less if you're not concentrating on it", it hurts less because you're not tensing up or fidgeting away from the needle, so the doctor/nurse/lab tech/whoever can put it in and take it out with almost no movement, contacting very few nerves.
When I was very small I got a needle for some reason or another, I thrashed about and it hurt like nothing else in existence.
When I was older, I needed to get blood drawn, and Mom told me "Try to observe it, carefully, scientifically" because I've always been hugely into math and science. And it worked--it hurt very little because I wasn't moving around, so instead of the needle shifting and hitting all manner of pain nerves, it hit only the ones it pierced going in and out.
