He's a doctor clown. He has tentacles on his back. Spiderman? Who's that? It was a failed DNA experiment. Occasionally the tentacles move about on their own. It's quite a bother when he isn't actively focusing on them, or keeping them wrapped around his torso when he goes out into public. The rare occasions that he does.
