It's actually a pretty straight-forward two-step process in Photoshop :) There's a Blur Gallery filter called Iris Blur that mimics how parts of an image may be less out of focus than the set focal point - you set where the focus is, and how much it blurs as it moves out. Then I just use the Add Noise filter to avoid that "overly smooth CG" look :)
