how do you get the ears to stick up like that? and did you just sew the [white/black] bits on a [black/white] background or what?

My pattern makes the ears stick up :)
And I'm not sure what you mean by that

meaning that it'd be fairly hard(nigh impossible) to find a cloth with that exact pattern that you can just cut out and do whatever hatty things you do, so how exactly do you get the pattern on the hat?

I just cut out the shapes, sew them onto each other, and then assemble the finished hat.
I.E. I wanted the stripes on the ears, so I cut out the ear bases and then sewed the stripes on, then put the ears together and sewed them onto the hat

Wow it's been 3 years since this was made?? Just wanted to pop in here again and say that I am still using it to this day regularely at school :D My teachers recognize me fast in the hallways!
