Two decades of hell made him see everyone and everything as a threat. His mind automatically focuses on the dangerous and unnatural aspects of someone. It's like fight or flight turned up to 11.
With angel here, he doesn't even register the actually nice and, dare I say, beautiful bits. All he sees is the screwed up bits. The multiple limbs, the pale fur, the sharp teeth, the many eyes ETC.
