This actually ended up being quite interesting to research! Assuming that Tom has his wet diaper changed on a regular sized changing table, then an online search reveals one of a similar design with a length measurement of 88cm/34.6", just under 3ft:

This is making some assumptions though. When he lands on the table he is slightly bunched up. When the anaesthetic is applied he slithers down and stretches out. We see his head is still on the table, but not if his booties are hanging off the other end!
