Neglecting to mark stuff read, I guess. If your Inbox is set to display newest submissions first, it's easy to ignore the older uploads if you don't scroll past the first thumbnail you remember seeing before.

I should add that by "people", It was actually one person whose laptop I looked at in his hotel room during a con. But the condition I laid out above, probably isn't a rare thing.
