Dracothrym are an ancient race, originally the larval stage of dragons. Fueled by vice, they required a constant influx of sin and stimulation to survive, such that most did not live long enough to hatch. Now, with the arrival of the Six Faced God, they can survive off the ambient hedonic magic that suffused the world, leaving them able to live a normal life.
When they breathe fire, the excess jets out around their eyes and through their ears and nose.

Also a fan of the leering bronze gargolye/goof faces and how colorful the dungeon is (differeniating the walls and floor like that looks really good)
Also also enjoying the idea of a creature that lives for the background radiation of mystic nastiness and impulsivity. If they show up when the world is full of that type of magic, will they be seen as a symptom of the grody magic permeating the world these days? Or possibly, by some, as potential champions of wholesomeness? Flame-spewing sin eaters?

Those are both probably ways different communities view dracothrym! To give a bit more setting lore, the way the world used to work was that living creatures had desires, needs, and fears, those generated magic, and that magic generated gods. The only way to do magic at that time was through ritual and prayer on the right days and times. The old civilization got so powerful and wise in the ways of the divine that it tried to build a tower to heaven... which promptly ruptured the thing and destroyed all the gods except for the oldest one on whose back heaven was built: Hex, the six faced God, built of the most ancient desires of the living. So, at the time of the ancients, it would be super rare to ever encounter one of these guys, but now that there's background vice radiation to spare, they're all over the place. This lady is a Lowland Dracothrym, who have built series of small villages throughout lowland valleys and swamps where they live as fisherman, merchants, musicians, dancers, and gardeners of psychoactive herbs; outsiders often view them as a sort of red light district culture, but I'd imagine dracothrym adventurers are quite popular hirelings in dealing with vice monsters!
A bit of both - outside the dungeon, where there's no Supernatural force keeping promising delvers alive so that it can feed on them, dracothrym have the advantage that vice monsters attacks can also help replenish their stamina. Much easier to not get killed by a greed monster when the greed monster powers up your fire-breath
