In my head canon, the lands around Tarkus' lair do have several human settlements.
It's a sort of symbiotic relation they have. Dragon protects and loosely governs the lands, leaving those that settle there more or less to themselves, only really demanding certain services, like a focus on life stock production and the occasional tribute delivered to his lair.
I suppose for a dragon like Tarkus it's something on the lines of a garden.
Sometimes you have to check your garden and see if everything is still in order, leading to unannounced surprise visits.
For example it's important to regularly examine, if the planning departments of the cities and settlements in your lands are keeping up with the current standards in terms of dragon accessibility.
Wide streets and sturdy roofs should be the bare minimum.

By the way, good topic you brought up dear Tarkus. I was wondering how the streets of Wales, Ljubljana (the capital of Slovenia), whose patron city is a green dragon, and Kazan itself are organised.

And the rest, yes, but Ljubljana and Cardiff love dragons, in the former there was a dragon animatronic on wheels in honour of an important holiday. In Kazan, unfortunately, there is no such thing yet, but they also love their serpent.
