The thing about Khan was he was intelligent, but not an experienced starship captain. He got hoodwinked by the prefix code, then ambushed in the nebula by a 3rd degree maneuver (Z minus 10,000 meters). Just because you are smart, does not mean you have the experience or training.
Indeed, but considering every trick Kirk pulls eventually bites him or even all of Starfleet in the tail eventually the higher ups would want to make sure somebody else doesn't use that trick against them.
Heck it was Kirk's fault Khan went on that rampage; first for thawing him out in Space Seed and then for leaving him on a planet and NEVER checking on him or notifying Starfleet about it. Also in the very next movie he hijacks the Enterprise to go on a Hail Marry mission to a forbidden planet to save Spock's soul. Not to mention all the times he nearly broke the timeline with reckless time travel shenanigans. Kirk is the reason for half of the changes to ship security and protocol over the next several decades. The Temporal Prime Directive we see brought up a lot in TNG and Voyager is probably 90% because of Kirk specifically.

Haven't seen that one in a while but I'm guessing it was shield frequency. Similar tactic was used by the Durass sisters in Generations when they planted a spy camera in Geordi's visor. Which...yeah that one's on shoddy security for not giving Geordi a full scan in Sickbay for anything like that after literally being a hostage on an enemy ship.
