Their true form is what they are. The little centipede creatures. Who take over people like parasites by inserting themselves on the spine (neck), connecting themselves to the nerves, killing and taking over the host. This can be any creature as long as they have nerves. Once done, they can edit and change their appearance to their liking. To answer your question they do not have to disguise themselves if they don't want to. It depends on who they are, where they are, and the social standards of the world or society they are part of.
For example, Mayoih does not like cities or towns, preferring to live in nature. So she is not very specialized in shaping her disguise, nor does she really care. It is not that a rabbit will look weird at her appearance if she goes to hunt it.
But for an example like Ōmukade, who is a Reborn that loves to be in cities and towns, surrounded by people. His speciality is manipulating people and affecting their behaviors. Which makes it critical for him to have a perfect disguise to fit in with the people. Obviously when the cat is out of the bag, he would reveal what he truly looks like. (I forgot to upload his Reborn appearance reference, but except for this artpiece I did of him.) But usually that means it is already too late to stop him and he basically got himself into some sort of powerful status.
The Reborn appearance itself just adds a few traits. Like horns, mandibles on the side, ears (with a unique colour type), and scales. Although it is something very small, Reborns do this out of instinct. Showing those traits is a form of imposing like animals do. These are pointed to their own kind to either tell them to 'back off' or to 'come challenge them' in a fight.

Thank you! And I have not. Currently as in the story, there are 12 left. (Each based on the Zodiac signs.) But there were many before them, they just.. died, got killed, etc.
The whole idea of the species is that they have a massive universe battle royal with each other. The winner being the one who can send their offspring into the next universe (Big Bang). So basically repeating the process over and over.
In a sense I could make and auction for some Reborns, saying they aren't part of that universe/storyline but of the universe before or next. I am one of those who likes to keep things lore accurate xD
