I promise you that does not only happen with hospital servers.
Pretty much any system that is not build with a failover (best in a different rack/building) in mind will sooner or later end that way.
In the past I worked in a DC where one of our provisioning team created works of art... half a year later that art was a mess of cables, due to changed hardware, changed cabling on customer request, etc
