As a GM, I've had each of these problems pop up at a game, just never all at the same time.

Funny story, no idea if it was true, but they described GMing a normal campaign and town and whatnot. Except the GM flipped the genders of all the NPCs. So it was this whole town full of women and almost non-existant men. And the GM could not convince the players there wasn't some plot happening. There's no cult with all the men trapped in a dungeon or something, it's just a normal town in a normal module. And one of the players was fixated on this side NPC because he was one of the only named males, a side characer who was like the spouse of the blacksmith or something, and utterlly convince he was somehow in on whatever was happening to the town.

You never know what will pop into a player's head, and once it's in there it can be next to impossible to get them to stop...
