When night comes Shadow has to chain himself to his bed so when he transforms into the Werehog he doesn't escape and kill people. He's also locked in a basement as sometimes the werehog breaks the chains. This is later stage of the disease, he IS pregnant here you just can't see due to the pose, give away is the extra fur (dewlap) around his neck.
