Not much of a story behind this one. Mainly just liked the idea. Kayko grew up in the wilderness with nothing/nobody in his early life, and this would be during that time. Hence why he doesn't have his cross. This is also before he had his growth powers.
The wilderness in question was once inhabited, and is still full of ruined towns and cities. Probably where he got his knife.
